Armory Square Veterans Walk of Honor

To assist you in providing the proper documentation for inclusion in the Walk of Honor, please attach a copy of one of the following verifications along with your application.

1. Copy of DD214 – Certificate of Honorable Discharge. The Washington County Veterans Service (740-568-9009) will assist you in obtaining these forms (discharge paper), Copy of Membership Card of any military club such as VFW, American Legion, etc., Copy of Certificate of Honorable Discharge. If you desire to honor a veteran from pre-WWII, a photo may help the VA obtain verification.
   
2. For young men and women currently serving, a copy of their military order will suffice. Please type the name of the honoree and their participation in the military. (See Examples) Note – Please double check spelling. Bricks will be engraved exactly as information appears on this form. Maximum 3 lines - 18 spaces per line. See examples below. Complete this form and mail along with verification and a check made payable to :
